QUOTE OR PAYOFF STATEMENT REQUEST FORM
On behalf of your Mortgage Lender, we are pleased to provide
you with a Reinstatement Quote or a Payoff Statement with respect
to your Mortgage Loan.
In order for us to process your request:
- All requests must be made in accordance with this Request
Form ONLY
- You must provide us with all of
the information in the Request Form, as applicable
- You must affirm proper completion, in the manner indicated
below.
If you are not the Borrower or the Borrower’s attorney,
you must fax us written authorization from the Borrower(s). NOTE:
If you do not fax us written authorization, we cannot honor your
request.

To Borrowers: Your Reinstatement Quote or Payoff Statement will
be mailed to the Mailing Address you provide below.
To Attorneys: Please indicate
the preferred manner of delivery for the Reinstatement Quote or
Payoff Statement.
NOTE: This is an attempt to collect a debt and any
information that you give us will be used for that purpose.
This communication is
not admissible in a court proceeding inasmuch as it relates to
a possible settlement negotiation. Your Mortgage Lender has instructed
us not to forbear from prosecution of any pending litigation
until reinstatement or payoff funds are received in accordance
with the terms of the Reinstatement Quote or the Payoff Statement,
as the case may be.
